CREATING A PATH FOR SUSTAINABLE CHANGE TO IMPROVE THE HEALTH AND WELL-BEING IN ELIZABETH
Building Healthy Connected Communities does not happen overnight. In order to ensure the Shaping Elizabeth Community Health Initiatives Team will endure we decided to craft our model for sustainability.
A Task Force of partners from Trinitas RMC, Groundwork Elizabeth, The Gateway Family YMCA and Rutgers Cooperative Extension developed a strategy and organizational structure for this purpose. It was then vetted with the entire team as well as with experts in the field of community development. Thank you to Janet Heroux, MHS, MBA, Prevention Consultant, Partnering for a Healthy New Jersey and Darrin Anderson, Sr. PhD, Deputy Director, New Jersey Partnership for Healthy Kids both Janet and Darrin are experts in working with communities in order to prevent Childhood obesity. Their insight and support for this project was invaluable. Attached is our strategy and structure which will support our continued development, growth and sustainability to serve the Elizabeth Community for healthy living.
We will share our plan at a Community Forum – “Why Should your Zip Code Be A Predictor of Your Health” on June 18, 2014. Attached is the Hold the Date which was sent to over 150 community leaders and residents. We are excited to engage more of the community in conversation about – Access to Healthy Food, Increased Physical Activity, School Wellness and Community Health. We are hopeful that after our Forum we engage many new partners in order to close the Gaps for Healthy Living for individuals and families in Elizabeth.
